Incident Disclosure 2025

Dropped Object on the SeaRose FPSO

March 10, 2025

On March 1, 2025 Cenovus reported an incident that occurred during cargo operations on the SeaRose FPSO.

The SeaRose FPSO experienced vessel motion after landing a container in the laydown area during lifting operations. The motion caused the crane hook to drift and contact a bumper rail, which in turn caused the pennant line assembly – consisting of a master link, wire rope and hook – to droop over a crash rail. This released the tension on the pennant line assembly, causing it to disconnect from the crane hook and drop to the deck below.

Crane operations were immediately suspended. The area had been barricaded and only authorized personnel were inside the exclusion zone. Due to the weight of the object and the distance travelled, the incident had the potential for fatality based on the Dropped Objects Prevention Scheme calculator.

Cenovus has initiated an investigation into the root cause of the incident, which the C-NLOPB is monitoring.
